INSURANCE WAIVER FORM

In order to play at J.A.G Airsoft you are required to complete and sign an insurance waiver for each person attending the site, please bring this along with you on the day.

 

If for any reason you are unable to complete the form prior to arriving at the site then there will be forms available to fill in when you arrive.
